Exercise Compliance in Physiotherapy.

If you have been provided with exercises from a physiotherapist and found it difficult to keep up with them, you’re not alone. Exercise programmes can be challenging to fit into busy schedules and adhere to when recovering from injury. However, exercise compliance is essential to improve function and also plays a crucial role in long-term outcomes and injury prevention. 

Understanding why exercise compliance is so important during recovery helps shift the focus from exercises feeling like a chore, to an essential aspect of your rehabilitation. 

What Is Exercise Compliance? 

Exercise compliance solely breaks down to how consistently somebody completes their prescribed exercises. This includes the frequency (number of sets and repetitions) and accuracy of the exercise. Good exercise compliance is demonstrated by consistently completing the programme to a good technical standard, in line with recommendations from your physiotherapist. Research has shown that patient adherence to exercise programmes is often less than 35% [1] despite the proven benefits exercise has to rehabilitation. 

Why Are Prescribed Exercises So Important? 

Other therapeutic modalities such as hands-on treatment and education play an important role in physiotherapy, but long-term improvements and outcomes rely heavily on active rehabilitation (exercises). In physiotherapy, exercises are individualised and selected to target your injury, movement patterns and goals; they are not random, they serve purpose for your recovery. Exercises help to: 

  • Regain muscular strength [2]
  • Improve control [3]
  • Re-load the tissues – especially for tendinopathy [4]
  • Reduce pain hypersensitivity [5]
  • Improve confidence in movement 
  • Reduce the risk of re-injury  [6]

The above are some of many reasons as to why exercise is beneficial to you when recovering from an injury. Without the remodelling and endurance under load which exercise provides, the body will not be able to perform effectively to the demands placed upon it.

The Role of Prevention 


Once recovered from the initial injury, it is important that exercise compliance is maintained as a preventative measure. Exercise therapy is a building block for maintaining the body’s strength in order to cope with the physical demands of life. This includes the demands of work, sport and daily life. The stronger we are and maintain ourselves to be, the less risk of re-injury [7]  therefore supporting long-term development. This is especially important for sporting individuals or individuals with a demanding occupation as falling back into old habits can allow previous problems to resurface – showing the power of a prevention-focused mindset. 

Barriers To Compliance

Remaining compliant to exercise programmes can be difficult. Common barriers we see to exercise compliance include: 

  • Lack of Time
    • Busy schedules can lead to difficulty in prioritising rehabilitation.
  • Access and space
    • Limited access to equipment such as weights and a set space such as a gym can make it difficult to replicate prescribed exercises.
  • Motivation
    • It can be easy to lose motivation, especially during a long rehab or chronic injury.
  • Pain
    • Exercise is not meant to be easy, however it can be hard to differentiate between an expected discomfort and pain. Some exercises can also aggravate symptoms. 
    • Pain can also decrease early in rehab, however this does not mean that the underlying issue has resolved, and so we must remain compliant. 
  • Fear
    • The fear of making things worse, in terms of the condition and pain, leads to fear avoidance of movement.
  • Confusion
    • Not understanding fully how or why to complete an exercise, means completion will not happen.

  • We educate our clients on pathologies and the benefits of exercise to avoid developing fear avoidance. 
  • We, where possible, link your exercises into your daily activities – when exercises are part of your existing habits, consistency improves. 
  • We progress exercises to keep rehabilitation engaging – this shows our clients improvement, building on their confidence and fueling motivation. 
  • We are always on hand to help in between appointments to help with any concerns you may have and modify programmes if you are suffering from an acute flare up.

Final Thoughts 

Exercise compliance is the key to achieving your rehabilitation goals and ensuring they are maintained. Compliance is built upon consistency, not short-term perfection. Creating habits that support recovery and bring you confidence in movement will assist you in the long term.
